  3. Available in Color formats

  4. Best printed  on 8 1/2 x 14 legal paper but will print on 8 1/2 x 11 letter size

Note: It has been reported by several people that they have been having trouble printing off these flyers. I have contacted Adobe and they have suggested that if you have trouble opening these forms to do the following. First locate your Adobe Reader in your programs folder and execute it.  Then once it is running click on the above link to the form.  They said this should allow you to open the form.  If you still have problems email me and I will help you any way I can.
